A Girl or Adult Health Information for Council Programs form must be completed and submitted for all girl participants and their Dads by Monday, 11/25. These forms found on our website, are “fillable” Word documents. To sign these documents, please type in your complete name on the applicable three signature lines. Attach the documents to an email and email them to [email protected].
